Fees for miscellaneous examinations, certificates and endorsements- (a) For a sight test for an examination in item 1 270 (b) For a further sight test on appeal 780 (c) For a further sight test on special appeal 1520 (d) For an examination of a candidate for an acting appointment as deck officer or marine engineer officer of a Hong Kong ship 7535 (e) For a certificate of service 890 (f) For a dangerous cargo endorsement to a certificate of competency or a certificate of service or a licence 515 (g) For endorsement of a certificate of competency, a certificate of service or a licence for the continued proficiency and updating of knowledge 530 (h) For a certificate of competency which is a Type Rating Certificate 150 7. Fees for copies of certificates or licences issued under this Part- (a) For a copy of a certificate of competency (other than a Type Rating Certificate), a certificate of service or a licence 220 (b) For a copy of a sight test certificate 150 (c) For a copy of a radar maintenance certificate 150 (d) For a copy of a certificate of competency which is a Type Rating Certificate 150 Note: No fee shall be payable under this item if the Authority is satisfied that the loss of a certificate or licence was without fault on the part of the holder of the certificate or licence. 8. Fees for a copy of any determination made under section 8, 10 or 15 of the Merchant Shipping (Seafarers) (Certification of Officers) Regulation (Cap 478 sub. leg.) 155 9. Fees in relation to certificates of proficiency in survival craft- (a) For sitting the examination for a certificate of proficiency in survival craft 515 (b) For issue of a certificate of proficiency in survival craft 150 (c) For issue of a copy of a certificate of proficiency in survival craft, except that no fee shall be payable if the Authority is satisfied that the loss of the original certificate was without fault on the part of the holder of the certificate 150 10. Fees in relation to certificates of competency as A.B.- (a) For sitting the examination for a certificate of competency as A.B. 515 (b) For issue of a certificate of competency as A.B. 150 (c) For issue of a copy of a certificate of competency as A.B., except that no fee shall be payable if the Authority is satisfied that the loss of the original certificate was without fault on the part of the holder of the certificate 150 (d) For issue of a copy of a certificate of competency issued under the revoked rules within the meaning of section 2 of the Merchant Shipping (Seafarers) (Certificates of Competency as A.B.) Rules (Cap 478 sub. leg.), except that no fee shall be payable if the Authority is satisfied that the loss of the original certificate was without fault on the part of the holder of the certificate 150 11. Fee for endorsement of validity of a certificate of competency, a certificate of service or a licence 155 12. For employment or discharge of seafarers where Form HK Eng. 2A is used- For recording the employment or discharge of any seafarer (L.N. 90 of 1999 and 44 of 1999 s. 43) Nil 4. For attesting crews' account of wages and performed- (a) at the Office and for each seafarer (b) other than at the Office and for each seafarer 160160 plus $535 per hour or part thereof on a work done basis. 5. For attesting a new clause in a crew agreement, if performed- (a) at the Office (b) other than at the Office 535535 plus $535 per hour or part thereof on a work done basis. 6. For miscellaneous services under this Part- (a) For issue of a certified copy of, or extract from, a document or statement of sea service- Each folio of 90 words or part thereof(b) For each sheet of a Rank Xerox (or similar) copy of a document provided by the Office (c) For inspection at the Office of a ship's expired official log book or crew agreement- Per set(d) For any search of the Office records at request of applicant (e) For the production by any officer of the Office of a ship's official log book or crew agreement in any court of law or at any inquiry on behalf of an applicant for the production thereof- |
