  (g) occupation, rank or profession of the missing person;
  
  (h) usual residence of the missing person at the time the person is found missing;
  
  (i) his nationality;
  
  (j) grounds for presumption of death; and
  
  (k) the making of a return of the missing person.
  
  31.
  
  In respect of a seafarer dying or missing while or after being employed in a ship, a record of whether he left any property (including money) on board the ship.

  Where an inquiry into the cause of a death is required to be held under section 122 of the Ordinance, a record-
  
  (a) of the name of the deceased and a reference to the relevant entry made under paragraph 29;

  33.
  
  Where a person employed in the ship falls ill or is injured, a record of-
  
  (a) the circumstances of the injury;

  (b) the nature of the illness or injury or the symptoms thereof;
  
  (c) the treatment adopted; and
  
  (d) the progress of the illness or injury.

  34.
  
  When a seafarer is charged with a disciplinary offence under the Disciplinary Offences Regulation, a record-
  
  (a) if the offence is not to be dealt with by the master, of the name of the officer authorized under section 6(2) of the Disciplinary Offences Regulation to exercise the powers and duties of the master in respect of that offence;
  
  (b) of the name of the seafarer charged;
  
  (c) of the charge;
  
  (d) that the master or the officer referred to in subparagraph (a) has read the charge to the seafarer;
  
  (e) (i) if the seafarer admits the charge, of a statement that he admits it; or
  
  (ii) in any other case, of a statement that the seafarer does not admit the charge;(f) (i) of particulars of any statement made by the seafarer in answer to the charge; or
  
  (ii) that he declines to make a statement in answer to the charge;(g) of the decision of the master or of the officer referred to in subparagraph (a)-
  
  (i) as to whether or not he finds that the seafarer has committed the offence charged; and
  
  (ii) if he finds that the seafarer has committed the offence, as to the amount of the fine which he is imposing for that offence or that he is imposing no fine; and(h) where subparagraph (g)(i) applies, that the master or officer referred to in subparagraph (a) has, in accordance with section 8(h) of the Disciplinary Offences Regulation-
  
  (i) informed the seafarer of his rights under section 9 of that Regulation; and
  
  (ii) supplied the seafarer with copies of all entries in the official log book (including any annexes thereto) referring to the offence charged or that the seafarer did not request such copies, as the case may be.
  
  The master or the officer named in an entry under subparagraph (a)

  35.
  
  If the master remits the whole or a part of a fine in accordance with the Disciplinary Offences Regulation, a record of the remission, referring to the relevant entry under paragraph 34 and stating the amount remitted and the reason for the remission.

  Where the Superintendent pursuant to section 9 of the Disciplinary Offences Regulation cancels entries relating to a disciplinary offence, a record of-
  
  (a) the cancellation of those entries;
  
  (b) the date of the cancellation; and
  
  (c) action taken to cause the fine concerned (if any) to be refunded.

  Where a fine has been imposed on a seafarer for a disciplinary offence-
  
  (a) a record that the amount of the fine so far as not remitted by the master has been paid to the Superintendent and-
  
  (i) identifying the seafarer on whom the fine was imposed; and
  
  (ii) specifying the amount paid; and

  (b) if the ship concerned is within the waters of Hong Kong, a record of the receipt by the Superintendent of the amount specified in the entry made pursuant to subparagraph (a)(ii).

  (a) all the particulars stated in the load line certificate issued under section 51, or the load line exemption certificate issued under section 63, of the Merchant Shipping (Safety) Ordinance (Cap 369) currently in force in respect of the ship and the positions of deckline and load lines;
  
  (b) the draught which would be shown on the scales on the stem and on the stern post of the ship if it were on an even keel and so loaded that the upper edge of the summer load line marked on each side of the ship were on the surface of the water.
